If you meet the eligibility criteria or are currently receiving long-term services and support, you may qualify for the transformative Community Health Choices (CHC) program.

Managed Care Organizations expertly coordinate your healthcare coverage and additional services tailored to your needs. Long-term services and supports (LTSS) are designed to assist you in managing daily living activities, allowing you to receive Nursing-Level Of Care in the comfort of your own home, rather than in a facility. This not only enhances your quality of life but also allows you to maintain your independence.
